NUTRITIONDEPARTMENT CARE NAVIGATOR/ASSESSOR FULL-TIME$12.00-$14.00 per hour based on experience 40 Hours per week, Primarily Day Shift/In-County Travel 90-Day Probationary Period; Opportunity for 90-Day & Annual Increase; Vision & Health Insurance Benefits, Paid Life Insurance, Paid Sick Days, Paid Holidays, Paid Vacation, and Paid Training & CEUsThe Care Navigator is responsible for delivering person-centered care navigation support within a community-based organization to frail, home-bound seniors that enhances home delivered meal services by standardizing and responding to health and safety screening assessments and observed changes that are conducted by drivers who interact with meal recipients and other staff on a daily basis. High School Diploma/GED, Minimum Bachelor’s degree in Social Work, Social Services, Nursing, or Gerontology, or related field, Minimum of 2 years Social Work, Social Services, LPN, or similar experience working with seniors and connecting seniors to medical and non-medical support services, Experience working with healthcare professionals a plus, Solid working knowledge of programs and services available in Guernsey County for senior citizens, Experience working with community-based organiza-tions, Sensitive to the needs of senior citizens, Must have excellent communication, written, presentation and interpersonal skills, Ability to interact and communicate effectively with project team, administration & staff, meal recipients, volunteers, community partners and collaborators, Ability to work independently and as part of a team, Good problem solving skills and critical thinking skills required, Data entry and computer literacy required, Ability to work with other forms of technology (e.g., software applications, database tools, mobile technology) preferred, Must be organized, attentive to detail, and possess a friendly and professional demeanor, Desire to make a positive difference in the lives of seniors. The Guernsey County Senior Citizens Center, Inc. offers a variety of services and recreational activities to all senior citizens aged 60 and older. Activities include quilting, bridge, crafts, bingo, monthly dinners, square dancing, line dancing, musical entertainment, holiday parties and much, much more. 

Social memberships are available to individuals beginning at age 50. There are no "set fees" for services provided to senior citizens aged 60 and better. All programs and services for senior citizens age 60 plus are provided on a "donation only basis" whereby no senior citizen will ever be denied services based upon their ability or inability to make a donation. Suggested & recommended donation amounts are available upon request. For seniors aged 60 and better the suggested donation for a meal is $2.50.The cost of a meal for those aged 59 & younger is $5.50 per meal.
